ABSTRACT

Trionycis Carapax is a commonly used Chinese medicine in clinical practice. Modern research on Trionycis Carapax mainly focuses on experimental research and clinical observation, which has been rarely reported in the literature. Based on the literature on medicinal herbs, medical books, prescriptions of all dynasties, this study carried out systematic textual research on the historical evolution and changes of the name, origin, producing areas, quality, efficacy, indications, processing methods, and contraindications of the Trionycis Carapax. As revealed by the textual analysis, the origin of Trionycis Carapax is Trionyx sinensis, and the carapace of T. steindachneri is not suitable for the preparation of Trionycis Carapax. The genuine producing areas of Trionycis Carapax include Yueyang, Jingzhou, southeast Anhui, and western Jiangsu in the middle and lower reaches of the Yangtze river. Regarding the quality, the number of ribs of Trionycis Carapax, such as seven ribs and nine ribs, is often used as the quality evaluation standard in ancient Chinese herbal books. However, through literature research and field inspections on the medicinal material markets, it is not advisable to take rib number as a quality evaluation criterion in modern times. With the change of the times, the efficacy and indications of Trionycis Carapax have gradually expanded on the basis of Shennong’s Classic of Materia Medica (Shen Nong Ben Cao Jing), and later generations widely apply it in internal medicine, surgery, gynecology, pediatrics, etc. It should be noted that the treatment of labor heat and bone steaming by Trionycis Carapax is derived from Shennong’s Classic of Materia Medica, not Treatise on the Nature of Medicinal Herbs (Yao Xing Lun) mentioned in ancient books such as Amplification on Materia Medica (Ben Cao Yan Yi). The processing methods of Trionycis Carapax are diverse, which are dominated by traditional vinegar processing. In terms of contraindications, Trionycis Carapax should not be compatible with bauxite and marble and is contraindicated in pregnant women. Those with spleen deficiency, weak stomach, and liver deficiency without heat should use it with caution. This study is expected to provide the basis for radical reform and further development and clinical utilization of Trionycis Carapax.

ABSTRACT

Endometriosis(EMs) is a stubborn gynecological disease caused by persistent immune-inflammatory effects, and is known as "benign tumor" because of its similar characteristics to malignant tumors. National physician master Professor BAN Xiu-wen believes that the spread of damp-evil is the pathologic foundation for inflammatory response of ectopic endometrium; accumulation of blood stasis is the pathological product of continuous inflammatory attacks, and the combination of dampness and stasis is the main pathogenesis for refractory EMs. Modern researches have shown that immune-inflammatory effect is the key mechanism for development of EMs, and is closely related to cell autophagy, all of which have made it become the hot spots in research of the pathogenesis, diagnosis and treatment of EMs. Therefore, with immune-inflammatory effect as the breakthrough point in this research, and with reference to the related research of autophagy, the correlation between "combination of dampness and stasis" and abnormal autophagy-induced immune inflammatory response in ectopic endometrium was discussed, to provide guidance for the clinical application of traditional Chinese medicine and modern research.

ABSTRACT

<p><b>OBJECTIVE</b>To establish a predictive scoring system which may serve for the prediction of sustained response to conventional interferon-alpha (IFN-alpha) treatment on chronic hepatitis B.</p><p><b>METHODS</b>A total of 474 IFN-alpha treated hepatitis B virus e antigen (HBeAg)-positive patients were enrolled in the present study. The patients' baseline characteristics, such as age, gender, aminotransferases, activity grading (G) of intrahepatic inflammation, score (S) of liver fibrosis, hepatitis B virus (HBV) DNA and genotype were evaluated; therapy duration and response of each patient at the 24th wk after cessation of IFN-alpha treatment were also recorded. A predictive scoring system for a sustained complete response (CR) to IFN-alpha therapy was established based on genetic algorithm. About 10% of the patients were randomly drawn out as the test set. Responses to IFN-alpha therapy were divided into CR, partial response (PR) and non-response (NR). The mixed set of PR and NR was recorded as PR + NR.</p><p><b>RESULTS</b>For the scoring system, the sensitivity and specificity were 78.8% and 80.6%, respectively.</p><p><b>CONCLUSION</b>This SCR scoring system has satisfying prediction efficiency and is easily employed in clinical practice. With this scoring system, practitioners can propose individualized decisions that have an integrated foundation on both evidence-based medicine and personal characteristics.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the technique and outcome of arthroscopic single-bundle reconstruction of posterior cruciate ligament (PCL) with quadrupled hamstring tendon.</p><p><b>METHODS</b>From April 2001 to October 2004, 49 knees with PCL tears in 49 patients were verified with arthroscope in this department. Of them, 13 were combined with anterior cruciate ligament tears, 14 with disruptions of the posterolateral corner, 6 with ruptures of the posteromedial corner and medial collateral ligament, 9 with lateral meniscus tears, 5 with medial meniscus tears and 2 with popliteal vascular tears. All the damaged PCLs were reconstructed with single-bundle of autogenous quadrupled hamstring tendons under arthroscope. Biodegradable interference screws or blunt titanium interference screws were used for direct anatomic fixation of the reconstructed ligament.</p><p><b>RESULTS</b>After operation, no severe complications occurred at early stage in the 49 patients. All of them were followed up for 10-52 months with an average of 22.0 months+/-10.7 months. Lysholm score was remarkably improved from 30-60 (mean: 47.96+/-8.16) preoperatively to 70-95 (mean: 89.08+/-6.10) at the last postoperative follow-up (P less than 0.01). Furthermore, there was a significant improvement in International Knee Documentation Committee (IKDC) score from abnormal (Grade C) in 10 knees and severely abnormal (Grade D) in 39 preoperatively to normal (Grade A) in 20, nearly normal (Grade B) in 24 and abnormal in 5 at the last follow-up. Of the 49 patients, 40 returned to the same activity level as before and 9 were under the level.</p><p><b>CONCLUSIONS</b>Single-bundle reconstruction of PCL with quadrupled hamstring tendons has the advantage of minimal trauma in surgery and satisfactory outcome.</p>

ABSTRACT

<p><b>INTRODUCTION</b>Problem-based learning (PBL), a pedagogic concept using a student-centred approach and problem-solving through small group discussions, has been adopted in varying degrees for years at all 11 medical institutes in Taiwan. Much evidence has shown that a number of factors can seriously affect student performance in PBL courses, such as the design of PBL scenarios, the tutors' character and students' attitudes and efforts.</p><p><b>MATERIALS AND METHODS</b>The aim of this study was to examine how the personal characters or knowledge base of Taiwanese medical students influence their performance in a hybrid-PBL curriculum. A total of 309 (234 male, 75 female) high-school entry undergraduate medical students participated in this survey. Self-assessed personal traits were presented in a 44-item questionnaire with a Big Five factor structure. Knowledge base was assessed by students' score point average (SPA) based on their previous 4-year education in medical school. Peer-assessed performance of students in the PBL curriculum was carried out using a well-developed, reliable and validated evaluation form.</p><p><b>RESULTS</b>Each student's peer-evaluated PBL performance can be divided into 5 principal components, which included control-lead, assist-coordinate, written organisation and compromise- comply (Eigen value >1). The consistency and reliability of the Big Five questionnaire on personal traits was analysed and discordant items were deleted (Cronbach's alpha = 0.72 to 0.86 after deletion). The relationship between the personal traits, knowledge base and PBL performance, as analysed by simple regression, showed that "extraversion" and "openness to experience" were positively related to the "assist-coordinate" characteristic in PBL performance, and "conscientiousness" was positively related to the "control-lead" characteristic in PBL performance. The SPA was positively related to the "assist-coordinate" or "control-lead" characteristic in PBL performance. The "agreeableness" was negatively correlated with the "control-lead" characteristic in PBL performance. After stepwise regression between the Big Five and each component of PBL performance, only the correlation between conscientiousness and control/lead, and between extraversion and assist/coordinate remained significant.</p><p><b>CONCLUSION</b>Knowledge and personality characteristics appear to be associated with student performance in a hybrid-PBL curriculum. The implications of this study on the future development and application of this assessment tool in medical schools are presented.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical characteristic of the multiple-ligament injured knee and evaluate the protocol, technique and outcome of treatment for the multiple-ligament injured knee.</p><p><b>METHODS</b>From October 2001 to March 2005, 9 knees with combined anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) tears in 9 patients were identified with clinical and arthroscopic examinations. Of them, 5 knees were combined with ruptures of posteromedial corner (PMC) and medial collateral ligament (MCL), 4 with disruptions of posterolateral corner (PLC), 2 with popliteal vascular injuries and 1 with peroneal nerve injuries. Six patients were hospitalized in acute phase of trauma, 2 received repairs of popliteal artery and 4 had repairs of PMC and MCL. Reconstructions of ACL and PCL with autografts under arthroscope were performed in all patients at 4 to 10 weeks after trauma, including reconstruction of PLC with the posterior half of biceps femoris tendon tenodesis in 4 patients and reconstructions of PMC and MCL with femoral fascia in 1 patient.</p><p><b>RESULTS</b>No severe complications occurred at early stage after operation in the 9 patients. All of them were followed-up for 10-39 months with an average of 23.00 months+/-9.46 months. Lysholm score was 70-95 with an average of 85.00+/-8.29. International Knee Documentation Committee (IKDC) score was from severely abnormal (Grade D) in 9 knees at initial examination to normal (Grade A) in 2 knees, nearly normal (Grade B) in 6 knees and abnormal in 1 knee at the last follow-up. Of the 9 patients, 7 returned to the same activity level before injury and 2 were under the level.</p><p><b>CONCLUSIONS</b>The multiple-ligament injured knee with severe instability is usually combined with other important structure damages. Therefore, careful assessment and treatment of the combined injuries are essential. Reconstructions of ACL and PCL under arthroscope, combined with repairs or reconstructions of the extraarticular ligaments simultaneously or in stages, have advantage of minimal trauma in surgery and satisfactory outcome.</p>

OBJECTIVE@#To develop a specific quality of life (QOL) scale for Chinese patients with benign prostatic hyperplasia (BPH).@*METHODS@#The scale was developed with the programmed decision methods. The item pool was certified by experts. Five methods were used in item selection after a pilot study for which 256 BPH patients had been recruited. The scale was evaluated by its reliability and validity.@*RESULTS@#We formed a 27-item quality of life scale specific for patients with benign prostatic hyperplasia prior test version (BPHSQL). The test-retest correlation coefficient and Cronbach's alpha coefficient of BPHSQL were 0. 774 and 0. 945. The structure of the scale was similar to the theory construction. The scale's correlation coefficients with criteria ranged from 0.531 to 0.700. BPHSQL could well discriminate the quality of life between BPH and non-BPH patients as well as patients with different degrees of symptoms, different sources and patients with or without urethral catheters.@*CONCLUSION@#BPHSQL is reliable, valid and sensitive, and will be a convenient tool in clinical research to provide advice on different treatments for different patients.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the role of Stat3 (one of the signal transductant and activator) signal transduction pathway in proliferation of vascular smooth muscle cells (VSMCs).</p><p><b>METHODS</b>VSMC was transfected with Stat3 antisense oligonucleotide. ELISA analysis was performed on Stat3 expression in cultured rat thoracic VSMC. Cell proliferation and toxicity assay by Methyl Thiazole (MTT) was used to measure the cell proliferation. The expression of Stat3, p-Stat3, Cyclin D1 and Bcl-x(L) were measured by Western blot.</p><p><b>RESULTS</b>Stat3 protein expression was positive in VSMC. Targeting of Stat3 using antisense oligonucleotide directed against the translation site, resulted in significant growth inhibition and downregulation of Stat3, p-Stat3 and Cyclin D1.</p><p><b>CONCLUSIONS</b>The increase of Stat3 correlates significantly with cell proliferation of VSMC. Cyclin D1 may be the critical target gene in mediating proliferation. Blocking Stat3 signal transduction pathway may inhibit the growth of VSMCs.</p>
